Whether you are suffering from chronic pain or looking for preventive healthcare solutions, the 360 Wellness Pass offers a 360 approach to health and wellness, without the guessing games and wasted time and money.
If you aren’t seeing results from your current treatment plan or are looking for an alternative solution to surgery or pharmaceutical drugs, the 360 Wellness Pass allows you to explore new, non-invasive treatments with a $500 in-house credit* that can be applied towards your initial visit and future treatments.
How it works:
STEP ONE: SIGN-UP FOR $500 CREDIT
Fill out the form below to sign-up and claim your 360 Wellnee Pass. You will receive an email with a temporary printable card, bring this to your initial appointment.
STEP TWO: ACTIVATE YOUR PASS
Once you are registered for the 360 Wellness Pass, all you have to do to activate your $500 in-house credit is schedule your appointment and meet with one of our Wellness Advocates with-in 10 days of claiming your 360 Wellness Pass.
STEP THREE: MEET WITH WELLNESS ADVOCATE
At your initial appointment, you will receive a a medical history consult along with one session of spinal or knee decompression. This evaluation is $97 and you can use $50 from your 360 Wellness Pass leaving only $47 out of pocket.
